About the role

This is an opportunity to join a newly established Birth to Settled Adulthood Service, working with young people and their families to support them as they approach adulthood.
 
You will be part of a team working in a strengths based multi-disciplinary way across health, social care, SEND and education. The role has a focus on preparation for adulthood from the age of 14  to 25 years. You will be completing Care Act assessments and care and support plans for young people along with other associated activities, such as completing MCA assessments/ Best interest meetings.
 
You will also take a role in  supporting colleagues with all aspects of preparation for adulthood work such as developing independence across all ages. You will be part of a peer group of professionals who are committed to proactively supporting Dorset young people and their families to achieve a smooth transition in a prepared and informed way without any gaps.
 
About you:
 
We are looking for someone who is an excellent communicator with a strong sense of professional curiosity and good decision making skills. Although experience in adult work would be beneficial if you have transferable skills and a desire to learn this post could be for you! 

Further Information

This is a part time time role at 22.2 hours per week.

This role will cover East Dorset. Your base can be agreed depending on where you live and what works for you with an opportunity to combine with working from home.

You must:

  • have a degree in social work/Occupational Therapy or equivalent
  • be registered with the Health and Care Professions Council (HCPC)
  • be able to fulfil the travel requirements of the post
