About the role

We have an exciting fixed-term opportunity to become part of Dorset Council’s innovative Birth to Settled Adulthood Service, dedicated to supporting young people and their families through their journey into adulthood.

About the role:

As part of our strengths-based, multidisciplinary team spanning health, social care, SEND, and education, your role will focus on supporting young people aged 14-25 as they prepare for adult life.

You will:

  • engage directly with young people and their families/carers to create personalised Preparation for Adulthood Plans.
  • support individuals to build independence and transition confidently into adult responsibilities.
  • undertake Care Act assessments, Mental Capacity Assessments, and participate in Best Interests meetings.
  • collaborate closely with colleagues across health, social care, SEND, and education to promote informed decision-making and independence.

About you:

We’re looking for someone who:

  • is an excellent communicator with strong interpersonal skills.
  • demonstrates curiosity and strong decision-making abilities.
  • has relevant experience or transferable skills and a willingness to learn.
  • ideally has experience in adult services, although this is not essential.

This post is fixed term for 12 months.

You must: 

  • have a degree in social work or equivalent 
  • be registered with the Health and Care Professions Council (HCPC) 
  • be able to fulfil the travel requirements of the post
