Carers are important members of our communities, providing unpaid support to family, friends and neighbours who would not be able to manage without their help.
An exciting opportunity has arisen for a part time Carers Case Worker position (26 hours a week) in the Adult Carers Team. This post will be geographically mobile and respond to increases in demand in different localities, as well as assisting hospital teams as necessary. As such, a willingness to travel across Dorset will be required along with suitable transport for this.
You will be based in a well-established, enthusiastic team, receive regular support from your line manager and have the support of an experienced team who work across Dorset. The post holder supports new and existing carers identified as requiring assistance and will hold a caseload and undertake strength-based Care Act (2014) carers assessments (recording these on our computerised client database), working with carers both within hospital settings and in the community.
The post holder will routinely complete support plans and reviews, all of which contribute to improving the wellbeing of the carer. Experience, skills, and knowledge, (including legal frameworks) of carers support is essential along with good computer literacy. The role includes providing emotional support and guidance to carers and their families, and working collaboratively and creatively with other professionals whilst remaining flexible.
