What You'll Be Doing
As a Social Worker in our Adult Social Care teams, your key responsibilities will include:
- Strengths-Based Assessments: Conducting assessments to understand the strengths and needs of individuals, helping them achieve their personal goals and promoting their independence.
- Support Planning: Developing and implementing support plans tailored to each individual's needs, ensuring they receive the appropriate services and resources.
- Reviews: Regularly reviewing support plans to ensure they remain effective and making adjustments as necessary to meet changing needs.
- Collaboration: Working closely with a multidisciplinary team, including healthcare professionals, community organizations, and other local authority teams, to provide comprehensive support.
- Promoting Independence: Encouraging and enabling individuals to be as independent as possible, supporting them in making their own decisions and managing their own lives.
- Legal Compliance: Ensuring all practices comply with relevant legislation, such as the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 2007, Human Rights Act 1998, and Equality Act 2010.
- Continuous Improvement: Engaging in continuous professional development to stay updated with best practices and new developments in social work.
